Full-Time Community Health Nurse Practitioner for US Veterans Health Care Clinic

Position Overview

We are seeking a dedicated and compassionate Nurse Practitioner to join our team at the US Veterans Health Care Clinic in Newburgh, New York. This full-time position offers an opportunity to provide holistic and comprehensive care to the veteran community, ensuring equitable access to quality healthcare services. As a key member of our healthcare team, you will work Monday through Friday, from 9 AM to 5 PM, delivering essential services that address the unique needs of our patient population.

Work Environment

This position is onsite, allowing for direct, face-to-face interaction with patients in a supportive home care setting. You will collaborate with fellow healthcare professionals and community organizations to enhance service delivery and engage with patients directly within their communities, making a tangible difference in their health outcomes.

Key Responsibilities

• Collaborate with community organizations to develop and implement tailored health education initiatives that empower patients and their families.
• Utilize clinical skills to perform comprehensive assessments, develop treatment plans, and deliver culturally sensitive care to each individual.
• Advocate for patients’ rights and ensure access to necessary healthcare services, providing a bridge between different care options and resources.
• Maintain detailed and accurate medical records in accordance with HIPAA and regulatory standards, ensuring confidentiality and integrity of patient information.
• Participate in community outreach events and health fairs, conducting screenings and educating the public about available health resources.
• Stay informed about the latest community health trends and evidence-based practices, and integrate this knowledge into patient care.

Patient Population / Client Focus

Your primary focus will be on serving active and retired military personnel and their families, who may face unique health challenges related to their service. This includes addressing both physical and mental health needs and providing a supportive, understanding environment that respects their experiences and sacrifices.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited program is required.
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) through the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP) is mandatory.
• Possession of both DEA# and NPI# is essential.
• You must demonstrate strong clinical assessment and diagnostic skills, with the ability to effectively communicate with a diverse patient population.

Preferred Experience

• A minimum of 2 years of clinical experience, preferably in community health or public health settings that cater to unmet medical needs.
• Experience working with veterans or in related healthcare environments is highly desirable.
• Strong interpersonal skills to build trust and rapport with patients and community stakeholders.

Compensation and Benefits

This position offers a competitive salary range of USD 150,000 to 160,000 per year, commensurate with your experience and qualifications.
