Key Responsibilities\n

• Collaborate closely with a team of healthcare providers, including physicians, nurses, and other social workers, to ensure comprehensive care that addresses each client’s individual needs.

• Conduct thorough psychosocial assessments, developing personalized treatment plans that consider not only medical conditions but also psychological and social factors affecting the client’s quality of life.

• Facilitate family meetings to discuss treatment goals, preferences, and support systems, ensuring that communication is clear and everyone involved understands their roles.

• Provide clients and their families with education about mental health resources and coping strategies to assist during the dying process, thereby enhancing their emotional resilience.

• Continuously review and monitor clients’ progress towards identified goals, adjusting treatment plans as necessary while maintaining accurate and up-to-date documentation.

• Stay current with best practices and emerging trends within the fields of social work and hospice care, participating in ongoing training and professional development opportunities.

Patient Population / Client Focus\n

This position specifically focuses on serving elderly populations and individuals facing terminal illnesses. You will work with clients who may be experiencing various challenges including grief, loss, and anxiety related to their health conditions or end-of-life decisions. With your expertise and empathy, you will advocate for clients' needs, helping them to navigate complex emotional landscapes while fostering dignity and comfort in their final days.

Qualifications\n

• A Bachelor’s degree in Social Work from an accredited institution.

• Must possess a valid and active LCSW license to practice in the state of Nevada.

• Demonstrated experience, preferably at least 2 years, successfully supporting clients dealing with end-of-life issues or working in hospice environments.

• Exceptional communication and interpersonal skills—ability to foster strong relationships with clients, families, and healthcare teams.

• Strong clinical skills, including the ability to assess and interpret complex client needs, providing supportive interventions accordingly.

Compensation and Benefits\n
