Key Responsibilities\n

• Provide direct clinical service as a social worker to assigned hospice patients, conducting thorough assessments and treatment planning tailored to their unique needs.

• Collaborate effectively with interdisciplinary teams to enhance patient care and develop comprehensive care plans that align with our holistic approach.

• Facilitate individual, family, and group therapy sessions, focusing on mental health challenges and emotional support for patients and their families during end-of-life transitions.

• Conduct timely and evidence-based screenings to identify and address any mental health concerns, ensuring appropriate referrals and follow-up treatments are enacted.

• Participate in ongoing case reviews, documenting progress, providing insights on treatment outcomes, and ensuring compliance with ethical and professional standards.

• Assist with crisis intervention as needed, utilizing de-escalation techniques while ensuring the safety and well-being of both patients and their families.

• Engage in community outreach to identify potential patients in need of hospice services and foster trust and connection with underserved populations.

• Maintain timely documentation of all patient interactions and treatment plans, ensuring adherence to regulatory and organizational guidelines.

In this role, you will work primarily with terminally ill patients facing complex emotional and psychological challenges. Qualifications\n

• A Master’s Degree in Social Work (MSW) or a related field is mandatory.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Maryland is required.

• Minimum of 2 years of direct clinical experience working with diverse populations, preferably in hospice or palliative care settings.
