Work Environment\n

As a Licensed Clinical Social Worker in a remote setting, you will have the flexibility to work from the comfort of your home while still being an integral part of our clinical team. Our work hours are Monday through Friday from 9:00 AM to 5:00 PM, allowing for a balanced work-life environment. Telehealth technology and virtual communication tools will be utilized to conduct sessions, ensuring you have the resources necessary to provide effective care and engage with patients meaningfully.

Key Responsibilities\n

• Conduct psychosocial assessments to determine patients' needs and develop personalized treatment plans, including discharge planning.

• Provide individual and group therapy sessions to address emotional and behavioral challenges faced by patients, utilizing evidence-based therapeutic techniques.

• Maintain accurate and timely documentation of patient interactions, clinical notes, and treatment progress in accordance with departmental policies.

• Communicate effectively with physicians and other healthcare professionals to report on patient progress, needs, and concerns, ensuring a cohesive approach to care.

• Offer crisis intervention services that include immediate assessment and support for patients experiencing acute emotional distress.

• Forge strong relationships with community resource organizations, attending meetings and participating in committees to advocate for the needs of our patients.

Qualifications\n

• A Master's degree in Social Work from an accredited institution, along with a current state licensure as a Licensed Clinical Social Worker (LCSW).
