The Senior Nurse Practitioner role is based in a hospital setting, providing direct, hands-on care to seniors. Key Responsibilities

• Provide comprehensive health education to residents and their families, enabling informed decisions about care plans.
• Develop, communicate, and adjust individualized treatment plans based on thorough assessments of residents' health conditions.
• Order and interpret diagnostic tests, such as blood work and imaging, and revise treatment strategies based on the results.
• Maintain meticulous and up-to-date documentation within our Electronic Medical Record (EMR) system, ensuring compliance with regulatory standards.
• Engage in collaborative discussions with the local and regional operations and clinical leadership to align on strategic objectives.
• Champion value-based care initiatives to enhance patient outcomes and satisfaction results.
• Create an exceptional customer experience to ensure high levels of resident and family satisfaction.
• Adhere to ethical standards and practices in accordance with professional and regulatory bodies, including Board Certification requirements.
• Cultivate strong partnerships with Primary Care Providers (PCP) and interdisciplinary care teams within the facility for comprehensive patient management.
• Coordinate care with specialists and caregivers, integrating telemedicine services where applicable.
• Conduct all mandated monthly and quarterly evaluations, addressing urgent care needs as they arise.
• Participate in family conferences, facilitating advanced care planning and educational discussions as necessary.
• Respond promptly to pharmacist recommendations regarding medication management and adjustments for residents.
• Ensure consistent onsite presence during work hours to fulfill patient care needs adequately.
• Assess, diagnose, and manage a wide range of chronic and acute medical conditions across the resident population.

Patient Population / Client Focus

This role primarily focuses on seniors, many of whom may have complex health needs requiring specialized care. The Nurse Practitioner will work collaboratively in a multidisciplinary environment to support geriatric patients, addressing their unique medical and psychological requirements while promoting overall well-being and enhancing quality of life within the community.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• National certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Active and unrestricted Nurse Practitioner state license and DEA license.
• Authorization to work in the United States.
• Ability to adapt to a dynamic work environment with a flexible schedule that accommodates patient needs.
• Demonstrated passion for providing healthcare to senior and underserved populations.

Preferred Experience

Candidates with previous experience in geriatric care or similar settings are preferred but not required. Familiarity with electronic medical record systems and a strong understanding of healthcare regulations and compliance are advantageous for this role.

Compensation and Benefits
