Work Environment

This role is primarily based in patients' homes, where you will deliver high-quality, one-on-one care. The home health environment provides a unique setting that allows therapists to interact with patients in their daily lives, tailoring interventions to real-world scenarios. You will work a Monday to Friday schedule, completing 40 hours a week, consistently engaging with patients in a warm and supportive setting.

Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians, analyzing their needs and establishing personalized treatment plans.
• Implement effective treatment regimes that address a range of issues from orthopedic injuries to neurological conditions, ensuring each intervention aligns with the physician's plan of care.
• Demonstrate exceptional customer service by engaging with patients and their families, helping them understand therapeutic processes and gains.
• Collaborate closely with a multidisciplinary team to achieve maximum patient outcomes, utilizing a variety of evidence-based methodologies.
• Provide education and training to patients and caregivers, equipping them with the necessary skills to support ongoing rehabilitation.
• Consistently meet established utilization goals and contribute to the overall success of the organization.

Patient Population / Client Focus

In this role, you will focus on a diverse patient population, including individuals recovering from surgeries, athletes rehabilitating from sports injuries, and pediatric patients receiving treatment for developmental challenges. Your expertise will help patients overcome mobility challenges, regain strength, and enhance their overall well-being in a familiar and comfortable setting.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program, with preference for candidates who hold a Doctorate of Physical Therapy (DPT).
• Current state licensure in Physical Therapy, along with certification in Basic Life Support from the American Heart Association.
• Excellent communication skills, both written and verbal, along with proficiency in using relevant software and documentation tools.
• Ability to build strong, therapeutic relationships with patients, fostering trust and encouraging them to achieve their functional goals.

Preferred Experience

• A minimum of two years of clinical experience in a physical therapy role, particularly in a home health or outpatient setting.
• Experience working with various patient demographics, including pediatric, geriatric, and those with complex orthopedic and neurologic conditions.
• Current CPR certification to ensure effective emergency response.

Compensation and Benefits
