Work Environment\n

This role will be conducted remotely, allowing for flexibility while upholding the highest standards of professional practice typical within a hospice care setting. Our team operates collaboratively, focusing on individualized care plans that meet the unique needs of each client and family. Regular virtual meetings and case reviews will ensure that all team members remain connected and informed about patient progress and needs.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of patients to develop tailored care plans that address physical, psychological, and social needs.

• Engage actively in the development and implementation of Individualized Family Service Plans (IFSP), ensuring that the family's priorities and concerns are reflected in care strategies.

• Monitor patient outcomes through ongoing evaluations and team collaborations, making necessary adjustments to care plans in response to changing patient circumstances.
\n
• Provide education and support to families regarding coping strategies, available resources, and the medical and emotional challenges faced during end-of-life care.

• Build strong rapport with families, fostering trust and open communication, integral to effective hospice care.
\n
• Ensure compliance with all Federal and State regulations governing hospice care and social work practices.
\n
• Utilize evidence-based practices to recommend strategies and interventions that respect the patient’s choices and autonomy.

• Facilitate team meetings to discuss patient care, share insights, and develop collaborative strategies for complex cases.

• Adhere to the health and safety standards to protect both staff and patients throughout the care process.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker specialized in hospice care, you will work directly with patients facing terminal illnesses, as well as their families, who often experience a host of emotional, social, and practical challenges. Your role will encompass supporting not only the patient’s well-being but also guiding family members through grief and caregiving processes. Qualifications\n

• A Bachelor’s Degree in Social Work is a minimum requirement; a Master’s Degree in Social Work or a Doctorate is preferred.

Compensation and Benefits\n
