Key Responsibilities\n

• Evaluate, diagnose, and treat a variety of psychosocial problems faced by patients and their families, utilizing individual clinical social work skills.

• Conduct comprehensive bio-psychosocial evaluations to identify barriers to treatment and enhance patients' health status, ensuring a personalized care approach.

• Develop individualized treatment plans employing both individual and group treatment modalities tailored to the unique needs of each patient.

• Collaborate with the multidisciplinary team to provide integrated care, ensuring effective communication regarding patient backgrounds and psychosocial needs.

• Document social work activities in patients' medical records according to departmental standards and compliance regulations.

• Deliver education and resources to help patients and their families understand and adhere to medical recommendations, contributing to improved health outcomes.

• Assist patients in navigating community resources and advocate for their social support needs, promoting self-determination.

• Stay up to date with available community resources relevant to patient needs, accessing them as appropriate.

Patient Population / Client Focus\n

In this role, you will primarily work with a diverse patient population experiencing a variety of mental health issues, including depression, anxiety, and other psychological disorders. Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is mandatory.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New York is required.

Compensation and Benefits\n

This position offers a competitive salary range of USD 85,000 – 100,000 annually, commensurate with experience and qualifications.
