Work Environment\n

This position offers a fully remote work setting, allowing you to manage your schedule while collaborating with a dynamic team of professionals. As a social worker working within a community health clinic framework, you will have the flexibility to provide services from the comfort of your own home, while also ensuring direct access to crucial resources and supports for families in need.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for children to identify developmental delays or disabilities.
\n
• Collaborate with families to create and implement Individualized Family Service Plans (IFSP) tailored to each child’s unique needs.
\n
• Monitor and assess progress on IFSP outcomes and make adjustments to interventions in alignment with developmental goals.
\n
• Engage families in a supportive partnership, equipping them with strategies and skills to enhance their child’s growth and development.
\n
• Utilize evidence-based practices in natural learning environments, ensuring that interventions are effective and meaningful.
\n
• Facilitate workshops or training sessions for families to promote community understanding of early development and intervention resources.
\n
• Maintain compliance with all federal and state regulations, ensuring high standards of care and safety are met for all clients.
\n
• Participate as a proactive team member in multi-disciplinary meetings to share insights on child development and family needs.
\n
• Demonstrate the ability to adapt treatment plans to suit different learning styles and preferences.
\n
• Provide continuous feedback and support to families regarding their child’s progress and developmental milestones.

Patient Population / Client Focus\n

You will be primarily working with infants and toddlers experiencing developmental delays or disabilities. The focus of our services is to support these young children and their families by nurturing essential development in areas such as physical, cognitive, social/emotional, and communication skills. Our holistic approach emphasizes partnership with families, ensuring they feel connected and empowered throughout the intervention process.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.
\n
• Current Licensure as a Licensed Clinical Social Worker (LCSW) issued by the Department of Public Health.

• A valid driver’s license and a reliable means of transport for possible fieldwork (if needed).

• Demonstrated expertise in early intervention practices and a commitment to improving child developmental outcomes.

• Excellent communication, organizational, and time management skills are essential in this role.

Preferred Experience\n

Compensation and Benefits\n
