This position is a full-time remote role, which means you will have the flexibility of working from home while being integrated into a supportive hospice center team. You will coordinate your work hours primarily Monday to Friday from 8:00 AM to 4:30 PM, with occasional on-call responsibilities during evenings or weekends as needed. This setup allows you to engage with our senior clients without the commuting stress, enabling you to focus on their well-being. Flexibility is paramount, as you may need to adjust your schedule based on client needs to ensure they receive continuous and prompt care.

Key Responsibilities\n

• Conduct comprehensive assessments to identify the psychosocial needs of seniors, considering factors such as emotional health, social circumstances, and living conditions.

• Collaborate closely with clients and their families to create individualized care plans tailored to their unique goals and preferences, ensuring that every decision is made with their input.

• Provide compassionate counseling sessions, helping seniors and their families navigate transitions, chronic illness, and loss, while supporting their mental health.

• Act as an advocate for clients by ensuring they have access to essential community resources, social services, and support networks.

• Work in conjunction with an interdisciplinary team of healthcare professionals—including nurses, caregivers, and medical staff—to ensure a holistic approach to client care.

• Regularly visit clients to monitor their progress, assess living conditions, and adapt care plans as needed to address any emerging concerns.

• Facilitate conflict resolution among family members involved in the client’s care and promote effective communication strategies.

• Educate clients and provide information about available community resources, support groups, and programs to enrich their social engagement.

• Maintain thorough, accurate documentation of client interactions, assessments, care plans, and relevant updates as per legal and ethical standards.

• Stay updated with best practices, policies, and trends in social work to continuously improve service delivery and compliance.

Patient Population / Client Focus\n

Your primary focus will be on seniors, often dealing with complex life situations that require a deep understanding of aging-related challenges. These clients may include those in hospice care, addressing not only the emotional adjustments related to terminal diagnoses but also the day-to-day stresses of aging. You will play a pivotal role in helping these individuals find comfort, dignity, and quality of life during their later years.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is essential.

Compensation and Benefits\n
