This position offers the convenience of remote work, allowing social workers to engage with clients from the comfort of their homes while maintaining a strong connection with our hospice team. Working Monday to Friday from 9:00 AM to 5:00 PM, you'll collaborate with an interdisciplinary team consisting of medical professionals, nurses, and support staff to ensure holistic care. Key Responsibilities\n

As an LICSW at Wings of the Future, you will:

• Conduct thorough assessments to evaluate the social, emotional, and psychological needs of patients and their families.

• Develop and implement comprehensive treatment plans tailored to each individual's unique circumstances and preferences.

• Facilitate family meetings and counseling sessions to enhance communication, resolve conflicts, and provide education on end-of-life issues.

• Coordinate with healthcare professionals to ensure that all aspects of care are aligned with patients' goals and values.

• Document progress and maintain accurate records within our electronic health record (EHR) system, ensuring compliance with regulatory standards.

Patient Population / Client Focus\n

Your primary focus will be on patients receiving hospice care, who may be experiencing significant emotional distress due to terminal illnesses. You will work closely with individuals dealing with grief, loss, and the challenges of transitioning towards end-of-life. By providing support and guidance, you will empower patients and families to engage in meaningful conversations regarding care preferences, advance directives, and emotional well-being.

Qualifications\n

To excel in this role, candidates must possess:

• A Master’s degree in Social Work from an accredited institution.

• Active LICSW licensure in Washington State with a clean disciplinary record.

• Extensive knowledge of clinical social work, including proficiency in treatment planning, diagnostics, and intervention techniques.

Preferred Experience\n

While two years of clinical social work experience is required, candidates with experience in hospice or palliative care, grief counseling, and crisis intervention are highly preferred. Familiarity with culturally sensitive practices to cater to a diverse client population is an added advantage.
