Key Responsibilities\n

• Conduct thorough assessments to identify various social, emotional, and environmental challenges faced by clients, ensuring a comprehensive understanding of their needs.

• Develop and implement tailored service plans that include clearly defined goals, interventions, and methods for tracking progress, ensuring that clients receive personalized attention.

• Provide counseling and emotional support to individuals and families, offering tools and strategies to cope with life’s hardships effectively.

• Engage clients actively in developing service plans, while connecting them to valuable community resources for additional support.

• Conduct crisis intervention when necessary, utilizing risk assessment and safety planning to ensure client well-being during critical times.

• Form partnerships with community agencies and organizations to facilitate coordinated services and advocate for clients' needs.

• Take part in community outreach efforts to elevate awareness of available agency services, which is essential for improving access to care.

• Participate in case conferencing, team meetings, and ongoing staff training to foster a collaborative team approach and enhance service delivery.

Patient Population / Client Focus\n

Your work will primarily focus on clients facing complex personal and social challenges, often within the context of a hospice care facility. You’ll provide critical mental health support to individuals and families during vulnerable and often challenging times. By employing compassionate communication and culturally sensitive methods, you will guide clients in navigating their circumstances and achieving a more robust support system through our clinic and community resources.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n
