Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews with patients, careful observation, and review of medical records to determine the emotional, social, and environmental factors influencing their recovery journey.

• Offer compassionate counseling and support to patients and their families, helping them navigate the emotional challenges of rehabilitation and adjustment.

• Collaboratively develop individualized treatment plans with the interdisciplinary team, focusing comprehensively on the patients' psychosocial needs and rehabilitation goals.

• Facilitate patients’ access to vital community resources such as support groups, vocational rehabilitation, and housing assistance, ensuring a holistic approach to their recovery.

• Educate and empower patients and their families regarding post-rehabilitation support and available community services, fostering a strong network of assistance.

• Assist in discharge planning, coordinating transitions to home or other care settings, and ensuring that patients have a comprehensive understanding of aftercare services.

Qualifications\n

Compensation and Benefits\n
