Key Responsibilities\n

• Facilitate engaging individual psychotherapy sessions and group therapy focused on coping strategies and emotional support.

• Deliver services in compliance with Path to Wellness, Inc. policies and established Mental Health and Substance Abuse standards while adhering to the ethical guidelines of social work practice.

• Perform comprehensive assessments of patients and their families, evaluating their social and emotional systems to identify specific needs and areas requiring additional support.
\n
• Develop individualized care plans in collaboration with clients and caregivers, establishing measurable goals and ongoing evaluations to track progress.
\n
• Continuously assess and modify treatment plans based on patient feedback and evolving needs.
\n
• Implement therapeutic interventions, including crisis counseling, to provide immediate support during challenging times.
\n
• Maintain accurate records of assessments, interventions, and progress to ensure continuity of care and adherence to legal documentation standards.

• Regularly communicate with healthcare professionals, including physicians, to coordinate comprehensive care for clients.

Patient Population / Client Focus\n

In this role, you will primarily work with patients facing severe and acute mental health challenges within a hospice context. Qualifications\n

Compensation and Benefits\n
