Work Environment

This position is based onsite, focusing primarily on home care facilities. You will have the flexibility to manage your schedule while working in diverse environments that foster strong community connections and relationships with your clients. Key Responsibilities

• Develop and execute individualized therapy plans for each patient, emphasizing early intervention and providing same-day evaluations.
• Conduct thorough patient evaluations and re-evaluations, demonstrating clinical accuracy in interpreting results and making informed decisions within your scope of licensure.
• Perform detailed chart reviews and collect patient data diligently to ensure continuity of care and effective progress monitoring.
• Collaborate seamlessly with a range of medical professionals, discussing patient histories and needs to optimize care plans and treatment outcomes.
• Maintain a comprehensive understanding of medically complex diagnoses, integrating relevant laboratory and radiology results while adhering to established contraindications and precautions.

Qualifications

• Active licensure as a Physical Therapist in the state of Florida.
• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
• Strong communication, interpersonal, and organizational abilities that facilitate effective interactions with patients, families, and healthcare teams.
• Capability to thrive in a fast-paced environment while managing varying patient caseloads efficiently.

Preferred Experience

• Experience in a home care setting or working with a diverse patient population is highly desirable.
