Work Environment

The Physical Therapist will conduct in-home visits, providing therapy in various environments, ensuring each patient receives tailored and effective treatment. Key Responsibilities

• Evaluate patients' physical conditions and medical history to develop personalized rehabilitation plans that focus on improving mobility and functionality.
• Implement therapeutic interventions using various techniques, including manual therapy, exercise regimens, and mobility training to aid recovery.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes and patient satisfaction.
• Thoroughly document all assessments, treatment plans, and patient progress in the electronic medical record system, maintaining compliance with legal and ethical standards.
• Educate patients and their families on exercises, wellness strategies, and preventative care to promote long-term health benefits.
• Work collaboratively with physicians, healthcare staff, and other treatment providers to coordinate comprehensive care for patients.
• Encourage patient engagement and motivation by setting achievable goals and celebrating progress towards improved health and mobility.

Patient Population / Client Focus

This role primarily involves working with orthopedic patients recovering from surgeries, injuries, or chronic pain conditions. You will provide care to a diverse clientele, including elderly patients who may require modifications to their therapy approach, ensuring that all care is respectful of their individual needs and circumstances. Your expertise will contribute significantly to their recovery, allowing them to regain independence and improve their quality of life.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• Current certification and state licensure as a Physical Therapist are mandatory.
• A clean professional record, free of any disciplinary actions or board actions against your license.
• At least 2 years of practical experience in a healthcare setting, preferably with a focus on orthopedic rehabilitation.

Preferred Experience

• Proficiency in developing tailored exercise regimens and rehabilitation techniques that meet each patient's unique needs.
• Experience in manual therapy and advanced therapeutic modalities.
• Familiarity with home care settings and the specific challenges they present in patient management.

Compensation and Benefits
