Work Environment

As an Onsite full-time Physical Therapist, you will primarily operate in the home settings of our patients, providing personalized and accessible therapy. Key Responsibilities

• Conduct comprehensive evaluations for patients, gathering pertinent medical history, performing a range of physical assessments, and determining actionable treatment recommendations.
• Effectively communicate evaluation findings and proposed treatment plans to patients and their families, actively incorporating their input in goal-setting processes.
• Implement individualized treatment protocols that are informed by patient assessments, health conditions, and current best practices in physical therapy.
• Collaborate with multidisciplinary teams, consulting with other healthcare professionals as needed to enhance patient care and adapt treatment plans based on progress or emerging concerns.
• Complete thorough discharge planning, ensuring appropriate follow-up care is in place. Document all patient care activities accurately and timely, including initial evaluations, daily treatment notes, and progress evaluations.
• Manage your schedule efficiently to meet productivity expectations, attending meetings punctually and participating in community-based marketing initiatives to promote health and wellness.
• Contribute to the development of departmental goals and adhere to regulation compliance for quality assurance and safety of practices.
• Engage proactively in continuing education and professional development opportunities for personal growth and organizational enhancement.
• Promote a superior customer experience through flexibility, knowledge, and responsiveness, ensuring patient satisfaction throughout their care journey.
• Participate in efforts to eliminate barriers to care, engaging actively in performance improvement initiatives and utilizing relevant data to drive access to therapy services.

Patient Population / Client Focus

This position will involve working with a varied patient population, including post-surgical rehabilitation clients, individuals dealing with chronic pain, athletes recovering from injuries, and elderly patients in need of mobility support. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program is required.
• Evidence of the necessary licensing to practice physical therapy in Florida.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

Compensation and Benefits
