Key Responsibilities\n

• Conduct comprehensive clinical assessments to evaluate patients' mental health needs, developing individual treatment plans based on assessment results.

• Deliver short-term psychotherapy and facilitate group therapy sessions that cater to the emotional needs of patients and family members dealing with end-of-life concerns.

• Provide expert consultation for counseling requirements, guiding interdisciplinary teams on best practices for patient-centric social work interventions.

• Plan and implement outreach initiatives to educate and engage the community regarding hospice care services and the importance of mental health.

• Participate in ongoing professional development opportunities, enhancing your skills and knowledge in specialized areas relevant to hospice and palliative care.

• Perform crisis assessment and intervention services, offering immediate support during mental health emergencies, while also assessing suicidal concerns and coordinating necessary hospitalizations as required.

• Maintain strict adherence to legal and ethical standards, ensuring that all therapeutic practices comply with the highest industry regulations and best practices.

• Engage with patients both in-person and remotely, as necessary, providing effective and empathetic care aligned with their individual needs.

Patient Population / Client Focus\n

As a Clinical Social Worker at our Hospice Center, you will work with a diverse array of patients, including adolescents and adults facing terminal illnesses. Your role will involve supporting individuals in processing grief, loss, and existential concerns related to end-of-life experiences. You will also guide families through this challenging journey, ensuring that they receive emotional support and counseling services tailored to their unique situations.

Qualifications\n

• A Master’s Degree in Social Work (MSW) and a current Licensed Clinical Social Worker (LCSW) designation is required. Consideration will also be given to licensed mental health counselors (LMHC) with equivalent qualifications.

• A minimum of 3 years of post-master's clinical experience in case management or mental health settings is essential.

• Strong background in managing treatment for diverse populations, particularly adolescents or college-aged individuals.

Compensation and Benefits\n
