This full-time role is essential in supporting individuals seeking mental health services within our community health clinic. This position operates remotely, providing you with the flexibility to work from the comfort of your home while connecting with clients across Keizer, Oregon, and surrounding areas. You will engage in virtual sessions and utilize telehealth technologies to offer effective therapeutic interventions. Key Responsibilities\n

• Manage and document the transition of clients from hospital or inpatient services to outpatient care, ensuring continuity of care through effective coordination with hospital staff during discharge processes.

• Utilize proven crisis de-escalation techniques in your daily routine to provide immediate support to clients, especially when serving as the Clinician of the Day.
\n
• Conduct comprehensive risk assessments and develop individualized safety plans tailored to each client’s unique situation, ensuring a proactive approach in crisis situations.
\n
• Maintain accurate and timely documentation of all interactions and assessments, ideally completing day-of documentation for individuals at risk to ensure a seamless report of services.
\n
• Submit critical incident reports within 24 hours of occurrence, maintaining compliance with organizational protocols.
\n
• Participate in after-hours care coordination support by carrying a phone on an alternating basis to address urgent client needs.
\n
• Perform additional duties as requested, remaining adaptable to the dynamic needs of our clientele.
\n
• Engage proactively with participants to prevent reliance on crisis services by encouraging the development of coping strategies and resources.
\n
• Track and report crisis alerts, ensuring smooth coordination with county crisis services for timely intervention.
\n

Patient Population / Client Focus\n

Your focus will be on a diverse clientele in Keizer and beyond, including individuals facing various mental health challenges such as anxiety, depression, trauma, and stress-related issues. By actively promoting their wellness and stabilization, you will help clients develop a stronger sense of agency over their mental health journeys.

Qualifications\n

• A Master's or Doctoral degree in clinical psychology, counseling, or clinical social work from an accredited institution. Acceptable degrees include MA, MS, MSW, MFT, MHC, PhD, or PsyD.

• A current and independent license to practice in the state of Oregon (LCSW, LMFT, LPC).
\n
• Proficiency in mental health practices, with strong analytical and critical thinking skills to evaluate situations and respond appropriately.

• An ability to communicate with clarity, maintain composure during high-stress situations, and provide empathetic listening.
\n
• Strong organizational skills, with the ability to prioritize tasks effectively and collaborate with colleagues to manage high caseloads.

Preferred Experience\n

• Previous experience in a community health clinic or similar environment working with diverse populations is preferred.

• Experience in crisis intervention and familiarity with telehealth practices are highly beneficial.
