This position is designed to be fully remote, allowing for flexibility while supporting the hospice team from your home office. You will collaborate with an interdisciplinary team of healthcare professionals, facilitating communication and ensuring that our patients receive the best possible care. The virtual setting complements the nature of the work, as you will primarily engage in assessments and counseling sessions through telehealth platforms and other digital communication methods.

Key Responsibilities\n

• Psychosocial Assessment: Conduct comprehensive assessments to identify psychosocial factors impacting patient care. You will utilize evidence-based practices to evaluate patients’ emotional, social, and financial needs, ensuring interventions are well-informed and timely.

• Clinical Intervention: Provide supportive counseling to patients and families. This includes assisting families in critical decisions, facilitating family conferences, and coordinating connections to community resources that can support their needs.

• Case Management: Efficiently manage a clinical caseload by identifying high-risk situations and collaborating with case managers for optimal discharge planning. You will balance ongoing patient care with incoming referrals, ensuring continuity of care.

• Documentation and Compliance: Maintain accurate and timely documentation in compliance with organizational policies. This includes creating Interdisciplinary Plans of Care and Social Services Progress Notes that reflect the care provided and support quality improvement initiatives.

• Training and Support: Mentor and supervise graduate social work students and MSW employees, contributing to their professional development through coaching and sharing your expertise.

• In-service Training: Conduct educational sessions for hospital staff to enhance understanding of the psychosocial aspects of patient care, focusing on real-life scenarios from your practice.

• Quality Improvement: Actively participate in initiatives aimed at improving the quality of hospice care by analyzing patient outcomes and suggesting program enhancements.

Patient Population / Client Focus\n

The LCSW will focus primarily on patients receiving hospice care, often facing complex emotional and psychosocial challenges related to serious illness and the end-of-life process. You will work closely with patients from diverse backgrounds, offering support in navigating their unique situations and enhancing their quality of life during this pivotal phase.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Minimum of 2 years of experience in a medical setting, with at least 1 year of experience in a cancer care environment strongly preferred.

• Current California Licensed Clinical Social Worker (LCSW) credential.
\n
• Basic Life Support (BLS) certification from the American Heart Association is required.
\n
• Fire Life Safety Training (must be obtained within 30 days of hire if not already certified).
\n
• Exceptional verbal and written communication skills in English.
\n
• Exemplary customer service skills and the ability to work collaboratively in a team setting.

Compensation and Benefits\n

The annual salary for this position ranges from USD 120,000 to 130,000 , commensurate with experience and qualifications.
