Key Responsibilities\n

• Collaborate with primary care providers to identify and manage mental health and psychosocial issues, acting as a vital member of the primary care team.

• Conduct patient intakes, performing comprehensive diagnostic and functional evaluations, then provide tailored recommendations regarding treatment goals and plans.

• Offer brief, focused interventions for patients needing immediate mental health support, fostering a trusting and supportive environment.
\n
• Evaluate patients’ needs to determine whether they can be adequately served in primary care or should be referred to specialty mental health services.
\n
• Initiate and maintain follow-up contact to monitor patient progress, assessing treatment efficacy and identifying necessary adjustments.
\n
• Help develop personalized relapse prevention plans and support patients in achieving stable functioning.
\n
• Recognize “at-risk” patients and collaborate on strategies to mitigate potential deterioration of their mental health status.
\n
• Document patient progress, treatment plans, and diagnostic information, ensuring comprehensive records that facilitate coordinated care.
\n
• Maintain ongoing communication with primary care providers regarding patient needs and progress to support collaborative treatment efforts.
\n
• Educate patients on self-management skills and provide relevant resources, fostering independence in their treatment and recovery journeys.

• Consult with clinic management and team members regarding behavioral health services, contributing to program evaluation and improvements.
\n
• Support patients in adhering to medical treatments initiated by primary care providers, including helping manage any side effects from medications.

• Abide by the Code of Ethical Conduct for Clinical Professionals and undertake other duties as assigned by management.

Patient Population / Client Focus\n

The counselor will work with a diverse patient population facing various mental health challenges. Patients may include those experiencing anxiety, depression, trauma, substance use disorders, and other psychosocial issues affecting their overall health. Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field is required.
