Key Responsibilities\n

• Conduct comprehensive assessments and ongoing evaluations to determine the mental health needs of your clients, tailoring interventions as necessary.

• Engage in clear and proactive communication with supervisors, colleagues, and clients' families to discuss progress and any required adjustments to treatment plans.

• Prepare and share a tentative schedule for patient appointments, including home visits and necessary observations to track individual progress.

• Manage home visits for comprehensive support, including observations and interviews, to facilitate in-depth understanding of clients' situations.
\n
• Actively participate in Interdisciplinary Team Meetings, contributing insights that enhance collective patient care strategies.
\n
• Document and complete monthly and quarterly progress notes with precision to support ongoing therapeutic effectiveness.
\n
• Coordinate and oversee training initiatives to ensure that staff are competent and confident in implementing program plans and maintaining accurate documentation.
\n
• Provide follow-up support through interviews, home visits, and additional observations to ensure continuous care for clients.
\n
• Maintain meticulous records to guarantee the availability of essential information for stakeholders and authorized parties.
\n
• Create a liaison relationship with the community and other mental health services to foster developmental support and resource sharing.

Qualifications\n
