Key Responsibilities

As a Physical Therapist at Heal & Revive Chiro Clinic, you will:

• Conduct thorough evaluations of patients' physical abilities, needs, and goals through diagnostic assessments, observational analysis, and detailed medical history reviews.
• Develop individualized treatment plans tailored to each patient's specific needs, utilizing evidence-based practices that target pain relief and enhance functional capabilities.
• Guide patients through therapeutic exercises, ensuring proper technique and form to facilitate recovery, while providing motivation and encouragement.
• Administer hands-on treatments, including manual therapy, traction, and the application of therapeutic modalities like heat and cold therapies.
• Actively supervise Physical Therapy Assistants, ensuring adherence to care plans and providing mentorship in best practices.
• Monitor patient progress diligently, documenting changes and adjusting treatment plans as necessary, while communicating effectively with the multidisciplinary care team.
• Engage with patients and their families to educate them on rehabilitation strategies, equipment use, and home exercises that support continued recovery.
• Participate in interdisciplinary team meetings to ensure a unified approach to patient care, sharing insights and discussing revisions to treatment plans as needed.

Patient Population / Client Focus

You will work with a diverse population, including individuals recovering from surgery, those with chronic pain, post-injury rehabilitation cases, and elderly patients seeking to enhance mobility. Our focus is on treating the whole person, not just their physical ailments, through comprehensive assessments and personalized care.

Qualifications

To be successful in this role, you must possess:

• A Doctorate in Physical Therapy from an accredited institution.
• A valid physical therapy license in New York State without any restrictions.
• Current CPR certification.
• At least one year of professional experience in a clinical setting, preferably in acute care or home health.
• Strong analytical skills paired with a creative approach to therapy, with a passion for patient advocacy.
• Exceptional interpersonal and communication abilities, with a knack for engaging clients and their families in their care journey.

Preferred Experience

While not mandatory, candidates with additional certifications in specialized techniques, such as orthopedics or geriatrics, will be highly regarded. Experience working collaboratively in interdisciplinary teams is also beneficial.
