Key Responsibilities

• Conduct thorough assessments of patients' health status, including physical examinations, medical histories, and relevant diagnostic tests.
• Formulate and implement individualized care plans based on evidence-based practices and patient-specific needs.
• Oversee the management of chronic diseases, providing ongoing evaluations and adjustments to treatment plans as necessary.
• Prescribe medications, therapies, and interventions in accordance with best practices and state regulations.
• Educate patients and families about disease prevention, medication adherence, and lifestyle modifications that promote health and well-being.
• Document clinical findings and care provided in electronic medical records to maintain accurate patient information.
• Engage in interdisciplinary team meetings to discuss patient progress and strategize care improvements.
• Participate in quality improvement initiatives aimed at enhancing the standard of care within the facility.

Patient Population / Client Focus

The Primary Care Nurse Practitioner will focus on a diverse population of adult and elderly residents, many of whom may present with complex medical histories and multiple chronic conditions. Qualifications

• A current certification as a Nurse Practitioner in the state of Florida.

• A minimum of 4 years of clinical experience, specifically in nursing or direct patient care roles.

Compensation and Benefits
