Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to pinpoint clients’ mental health needs, strengths, and challenges, tailoring your approach to individual circumstances.

• Deliver individualized, family, and group therapy sessions based on best practices, including evidence-based approaches such as cognitive-behavioral therapy and motivational interviewing.

• Develop and implement personalized treatment plans that focus on goals, interventions, and ongoing monitoring of client progress.

• Engage with a multidisciplinary team, including psychiatrists and nurses, to formulate cohesive treatment strategies and ensure holistic care for clients.

• Provide crisis intervention support, conducting risk assessments and safety planning when necessary to mitigate immediate concerns.

• Act as an advocate for clients' rights, connecting them with vital community resources and providing case management to enhance their access to care.

Patient Population / Client Focus\n

In this role, you will work primarily with individuals and families experiencing various mental health challenges in the context of hospice care. • Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n
