Key Responsibilities\n

• Conduct comprehensive assessments and regular evaluations of client needs to tailor personalized treatment plans.

• Facilitate effective communication with families, caregivers, and interdisciplinary teams to provide consistent updates on client progress.

• Maintain meticulous records and share treatment plans with supervisors, which include scheduling appointments and tracking observations.

• Organize virtual home visits when necessary to gather insights on clients’ environments and progress.

• Ensure the successful execution of program strategies while adhering to ethical guidelines and legal standards related to mental health care.

• Participate actively in Interdisciplinary Team Meetings, sharing insights and contributing to holistic care approaches.

• Compile and present monthly and quarterly progress reports to monitor client outcomes effectively.

• Develop and lead training sessions for team members to enhance proficiency in program delivery and documentation.

• Provide ongoing support through follow-up sessions and home assessments, ensuring that client needs are met consistently.

• Serve as a liaison between the community and mental health resources, helping individuals reach their developmental goals.

• Participate in the development of training materials aimed at enhancing program execution.

Patient Population / Client Focus\n

The LPC will work with a diverse client population, including both adults and youth, who are facing a range of challenges such as mental health issues, substance misuse, and trauma. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted LPC license in the state of Colorado.

• A minimum of two years of counseling experience in a clinical setting.
