Work Environment

You will be working onsite, directly within patients' homes, providing a unique work environment that allows for personalized patient interactions and tailored care plans. Our clinic's home care model ensures a diverse range of therapeutic practice, allowing you to develop strong relationships with patients while assessing and addressing their specific needs in a familiar setting. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions, including reviewing medical history and performing specific assessments to identify rehabilitation needs.
• Develop and communicate individualized treatment plans that incorporate patients' personal goals and relevant therapeutic objectives.
• Implement therapeutic interventions consistent with evidence-based practices, adjusting treatment protocols as necessary to reflect the latest healthcare trends and patient responses.
• Collaborate with a multidisciplinary team, including physicians, nurses, and occupational therapists, to ensure coordinated and effective patient care.
• Maintain accurate and thorough documentation of patient care activities, contributing to departmental goals and ensuring compliance with health regulations.
• Engage in continuous professional development opportunities, including attending workshops and serving as a clinical instructor for aspiring physical therapists.

Patient Population / Client Focus

Our clientele primarily includes adults and seniors who require physical rehabilitation following surgical procedures, injury, or chronic illness. Many of these patients seek therapy to improve mobility, manage pain, or regain independence in their daily activities. You will have the opportunity to positively impact their recovery journey by offering tailored interventions and emotional support. Qualifications

• Hold a Doctorate in Physical Therapy (DPT) and possess a valid DPT license in the State of Florida.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is required.
• Ability to demonstrate excellent communication skills, both verbally and in writing, to facilitate collaboration with the healthcare team and effectively educate patients and families.

Preferred Experience

• While a minimum of 2 years of clinical experience is preferred, we are open to candidates who exhibit a strong desire to learn and contribute in a home care environment.
• Experience in geriatric care or orthopedic rehabilitation is an asset, but not mandatory.

Compensation and Benefits
