Key Responsibilities\n

• Engage in ongoing communication with physicians and other healthcare providers to report on patient progress, address needs and concerns, and identify potential and actual problems affecting mental health.

• Maintain comprehensive and accurate clinical documentation, including progress notes and treatment plans, in alignment with departmental policies and regulatory requirements.

• Participate in interdisciplinary treatment planning sessions, contributing to the development of individualized care plans and discharge strategies based on psychosocial assessments.

• Provide crisis intervention and counseling services, employing evidence-based therapeutic techniques to assist clients experiencing emotional and behavioral difficulties.

• Coordinate effectively with clients and their families to facilitate seamless discharge planning and access to ongoing support resources within the community.

• Monitor and follow up on client progress post-discharge, ensuring continuity of care and successful integration back into their support systems.

Patient Population / Client Focus\n

You will work with a diverse patient population, including individuals from various cultural backgrounds facing mental health challenges such as anxiety, depression, trauma, and relationship issues. Qualifications\n

• A Master's degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
