Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the emotional, functional, and social needs of patients and their families.

• Develop and implement personalized treatment plans that incorporate evidence-based intervention strategies aimed at relieving emotional distress.

• Utilize solution-focused therapies to guide clients through grief, anxiety, depression, and other challenges associated with terminal illness.

• Maintain meticulous clinical records while ensuring compliance with HIPAA regulations and safeguarding the confidentiality of patient information.

• Actively collaborate with the treatment team to discuss patient progress and modify care plans as needed.

• Provide education and support to patients and families, facilitating discussions about care options, prognosis, and emotional health management.

• Stay informed about the latest developments in mental health, including new therapeutic approaches and industry best practices.

Patient Population / Client Focus\n

You will primarily work with patients receiving hospice care, along with their families, providing emotional support during one of life’s most challenging times. Your clinical expertise will assist them in navigating complex feelings related to grief, loss, and acceptance, contributing significantly to their mental well-being and quality of life.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid and active LCSW license in the state of Illinois.

• A minimum of three years of clinical experience specifically in psychotherapy, preferably in a healthcare setting.

• Strong proficiency in solution-focused and evidence-based therapeutic interventions.

• Excellent communication skills, both verbal and written, with a focus on empathetic patient interaction.

Compensation and Benefits\n

Our competitive salary range for this position is between $85,000 and $105,000 annually, with the potential for a sign-on bonus.
