Key Responsibilities\n

Your responsibilities as a Licensed Professional Counselor will include:

• Conducting comprehensive assessments of patients to understand their specific needs and develop individualized treatment plans.

• Maintaining open lines of communication with supervisors, colleagues, family members, and guardians to facilitate patient progress and ensure a coordinated approach to care.

• Providing a structured schedule to supervisors outlining your appointments, home visits, and therapy sessions for effective monitoring of client advancements.
\n
• Facilitating essential home visits that comply with observation standards, enabling in-depth case analysis and support.
\n
• Implementing and adapting program plans effectively, ensuring that clinical approaches are both efficient and evidence-based.
\n
• Participating actively in interdisciplinary team meetings, contributing your insights to enhance patient care strategies.
\n
• Documenting client progress through monthly and quarterly notes to track developments and plan future interventions.
\n
• Organizing training sessions to help enhance the competencies of team members concerning program implementation and documentation practices.
\n
• Conducting follow-up interviews and home visits to ensure ongoing support and to address any evolving patient needs.

• Acting as a liaison between the organization and the community, helping to build cooperative relationships that support developmental goals.

• Assisting in the formulation of training materials to enhance program efficacy and team training.

Patient Population / Client Focus\n

You will serve a diverse clientele, including both adults and youth with a wide range of mental health challenges. Your focus will be on providing evidence-based therapeutic support to individuals struggling with various issues such as anxiety, depression, trauma, and life transitions, particularly in the context of family dynamics and substance misuse.

Qualifications\n

To be successful in this role, you must meet the following criteria:

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• An active and unrestricted license as a Professional Counselor (LPC) in the state of Arizona.

• A minimum of two years of counseling experience, preferably in a clinical setting.

• A strong comprehension of ethical and legal standards in mental health practice.
\n
• Excellent communication and interpersonal skills, enabling rapport-building with clients from diverse backgrounds.

• Proven collaborative skills, demonstrating your ability to work effectively within a healthcare team.

• Familiarity with electronic medical records (EMR) software and associated technologies.

Preferred Experience\n
