As a Home Health Physical Therapist, you’ll work one-on-one with patients recovering from injury, surgery, illness, or chronic conditions. Your expertise will help improve strength, balance, mobility, and overall quality of life through personalized treatment plans designed around each patient’s unique goals and needs.

What You’ll Do:

• Evaluate patients in their home environment and develop individualized treatment plans

• Provide hands-on physical therapy treatments to improve mobility, strength, balance, and function

• Educate patients and families on exercises, safety techniques, and recovery goals

• Monitor patient progress and adjust care plans as needed

• Collaborate with physicians, nurses, caregivers, and interdisciplinary healthcare teams

• Maintain accurate and timely documentation in compliance with healthcare standards

What You’ll Bring:

• Active Physical Therapist license

• Home health or rehabilitation experience preferred

• Strong communication, time management, and patient care skills

• Ability to work independently while managing a flexible daily schedule

• Compassionate approach to patient-centered care

Requirements:

• Current CPR or BLS certification required

• Valid driver’s license and reliable transportation required

• Ability to travel to patient homes within the assigned service area
