Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing medical histories and performing assessments to create individualized treatment plans tailored to specific needs.
• Communicate effectively with patients and their families, explaining evaluation findings and incorporating their goals into the treatment plan.
• Implement treatment protocols proficiently based on assessment findings and adapt them as necessary in response to the patient's evolving condition.
• Collaborate with interdisciplinary healthcare teams to enhance patient care, documenting changes in condition and necessary adjustments in the care plan.
• Complete discharge planning and follow-up care while maintaining accurate documentation that captures the complete patient care journey from initial evaluation to discharge.
• Demonstrate effective time management by maintaining an accurate schedule, ensuring punctuality, and fulfilling productivity expectations.
• Contribute to achieving departmental goals by adhering to quality standards and participating in community marketing efforts.
• Engage in ongoing professional development and continuing education to enhance skills and share knowledge with peers and students.
• Provide a compassionate and supportive environment for patients to foster recovery and rehabilitation.

Patient Population / Client Focus

As an Onsite Physical Therapist, you will work with a diverse patient population, including elderly individuals, patients recovering from surgery, and those with chronic conditions. Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution is required.
• Valid Physical Therapy license in the state of Florida.
• American Heart Association BLS for Healthcare Providers CPR certification.

Preferred Experience

• Although prior work experience is not mandatory, a minimum of 2 years in a clinical setting is preferred.
• Experience working in home health or outpatient settings will be advantageous.
