Work Environment

The schedule for this role is primarily Monday through Friday from 2 PM to 8 PM, allowing for a work-life balance that benefits both you and our patients.

Key Responsibilities

• Provide high-quality, patient-centered care by managing the therapeutic process for an assigned group of patients in coordination with physicians and the patient care team.
• Conduct thorough patient assessments to develop and implement individualized care plans that emphasize goal attainment and patient engagement.
• Deliver evaluations and treatment interventions in compliance with established professional guidelines and standards of care.
• Assess and evaluate patients' therapeutic and functional status, contributing to the total plan of care.
• Provide therapeutic interventions at patients' homes as necessary, utilizing effective procedures and techniques to drive positive outcomes.
• Educate patients, families, and caregivers in the use of therapies and home exercise programs to promote ongoing recovery and wellness.
• Prioritize and manage your caseload efficiently while coordinating staff assignments and ensure smooth hand-offs during patient transitions.

Patient Population / Client Focus

The patients you will serve include individuals recovering from orthopedic surgeries, those diagnosed with chronic conditions, as well as pediatric patients requiring tailored rehabilitation interventions. Our focus extends to enhancing the quality of life for individuals grappling with mobility issues, pain, and functional limitations. You will be instrumental in helping each patient achieve their rehabilitation goals and improve their overall health and wellness.

Qualifications

• Active Florida Physical Therapy License.
• Bachelor's or Doctorate degree in Physical Therapy from an accredited institution.
• Basic Cardiac Life Support (BCLS) certification must be obtained within 30 days of employment.

Preferred Experience
