Key Responsibilities\n

As a Licensed Clinical Social Worker at CHDC, your primary duties will encompass:

• Providing clinical behavioral health services in alignment with the health center's mission, ensuring that all patient interactions adhere to our quality standards.

• Assisting mental health consumers in navigating both physical and behavioral healthcare systems to encourage integrated, holistic health solutions.

• Educating patients and communities on the available resources and services to enhance awareness around disease prevention, wellness, and healthy lifestyle choices.

• Participating actively in multi-disciplinary treatment team meetings, contributing insights and recommendations for patient care strategies.
\n
• Promptly reporting all crises and unusual incidents to the Medical Director, with immediate escalation of psychiatric and medical emergencies to the Chief Medical Officer (CMO).
\n
• Maintaining detailed and accurate documentation within each patient’s clinical record, ensuring compliance with healthcare regulations and organizational protocols.
\n
• Engaging in quality improvement initiatives as assigned, including attending relevant meetings and training sessions to continuously enhance your skills.
\n
• Upholding all organizational policies and procedures with the utmost professionalism, reflecting our commitment to high-quality care.
\n
• Utilizing evidence-based screening tools to assess and evaluate the healthcare needs of individuals effectively, reporting findings as required.

• Collaborating with our Center of Excellence Team to support patients with Opioid Use Disorder, fostering a supportive environment for recovery and health management.

Patient Population / Client Focus\n

This role primarily serves a diverse population of patients within the hospice framework, addressing both physical and mental health needs. Our focus on integrated healthcare means you will be working alongside healthcare professionals to build a cohesive treatment plan that responds to the unique challenges faced by individuals with serious illnesses and their families. Your work is pivotal in promoting a peaceful and dignified end-of-life experience for patients.

Qualifications\n

• A minimum of a Master’s degree in Social Work is required, along with a valid Licensed Clinical Social Worker (LCSW) designation.

• At least 2-3 years of direct experience in a behavioral health setting, preferably within a medical environment where collaborative work with other healthcare providers is essential.

• Your experience should include familiarity with the integrated care model and a willingness to engage in ongoing education and training.

• Strong interpersonal skills and the ability to build rapport with patients, families, and interdisciplinary teams are crucial.

• Willingness to travel to CHDC sites for meetings, retreats, and outreach is necessary.

Compensation and Benefits\n

The salary range for this position is competitive, ranging from USD 90,000 to 100,000 annually, commensurate with experience.
