Key Responsibilities

• Examine and evaluate patients to develop tailored treatment plans aimed at addressing functional limitations and disabilities.
• Provide ongoing consultation to healthcare providers regarding patients' treatment plans, ensuring continuity of care across the healthcare spectrum.
• Interact with patients from diverse backgrounds—regardless of age or culture—adhering to our standards of exceptional customer service and collaborative care.
• Conduct detailed evaluations, implement effective treatment strategies, and develop discharge plans while educating patients on their conditions and treatment options.
• Monitor and assess the effectiveness of treatment plans, making necessary adjustments in compliance with applicable laws, regulations, and professional documentation standards.
• Communicate effectively with the interdisciplinary treatment team, referring physicians, and other stakeholders to enhance overall patient care.
• Engage in community outreach initiatives and develop programs aimed at improving health awareness and rehabilitation practices.
• Stay current with continuing education opportunities to enhance your skills and maintain compliance with documentation standards.

Patient Population / Client Focus

Our patients range from recovering athletes to elderly individuals requiring post-surgical rehabilitation, allowing for a diverse and dynamic case load. You will work closely with patients recovering from a variety of conditions, including orthopedic injuries, neurological disorders, and chronic pain syndromes, customizing treatment plans to meet individual needs.

Qualifications

• A Bachelor's Degree in Physical Therapy is required.
• A valid State License in Physical Therapy and current BLS certification are mandatory.
• Proficiency in written and verbal communication skills, alongside knowledge of computer systems for documentation and workflow processes.

Preferred Experience

• At least two years of clinical experience in a physical therapy setting.
• Experience working with an interdisciplinary team to optimize patient care.
