Key Responsibilities\n

• Conduct thorough assessments to determine clients' social, emotional, and environmental needs and challenges.

• Create comprehensive service plans that outline goals, interventions, and methods for progress monitoring tailored to individual client needs.

• Provide individual and family counseling, focusing on strategies to navigate life challenges, including addiction, trauma, and social barriers.

• Engage clients in the development of personalized service plans and facilitate connections with appropriate community resources.

• Implement crisis intervention strategies, offering risk assessments and safety planning for clients experiencing acute distress.

• Work collaboratively with community agencies and organizations to merge resources and advocate effectively for clients' holistic needs.

• Participate in community outreach initiatives, providing education to raise awareness about the agency’s services and advocate for mental health issues.

• Attend case conferences, team meetings, and continued professional development sessions to enhance team collaboration and service delivery.

Patient Population / Client Focus\n

The primary client population consists of individuals in the detoxification process who may be grappling with substance use disorders. Many clients may also confront co-occurring challenges such as mental health disorders, domestic violence, homelessness, and poverty. The LCSW will offer compassionate and informed care aimed at empowering clients through their recovery journey, with an emphasis on establishing long-term stability and health.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
