Key Responsibilities\n

• Conduct in-depth psychosocial assessments to identify patients’ and families’ needs, strengths, and any barriers to accessing care.

• Provide crisis intervention and short-term counseling during acute distress, guiding families through complex emotions and choices.

• Facilitate communication between patients, families, and the healthcare team to promote understanding and coordination of care.

• Offer ongoing supportive counseling from the moment of diagnosis through the progression of the illness and into bereavement as necessary.

• Educate patients and families about available community resources and coping strategies to enhance support networks.

• Document all interactions and interventions accurately, ensuring compliance with organizational policies and maintaining confidentiality.

• Participate in interdisciplinary meetings to provide insights on the psychosocial aspects of patient care and collaborate on comprehensive care strategies.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) license in the state of Pennsylvania.

Compensation and Benefits\n
