As part of our Hospice Center team, you will collaborate with a multidisciplinary group of professionals, including psychiatrists and psychologists, to offer coordinated and holistic care.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to understand clients' mental health concerns, strengths, and available resources.

• Provide individualized therapy sessions, including one-on-one, family, and group therapy, employing evidence-based treatment modalities appropriate for hospice care.

• Develop and implement personalized treatment plans that align with clients' goals and include measurable interventions.
\n
• Facilitate group therapy sessions, psychoeducation classes, and support groups to foster community and shared healing.

• Collaborate with other team members to integrate client care, ensuring that all aspects of a client's well-being are addressed.

• Deliver crisis intervention services when required, including risk assessment and safety planning to effectively navigate acute situations.

• Maintain thorough and accurate documentation of client interactions and treatment progress, adhering to all ethical and legal standards.

Patient Population / Client Focus\n

The successful candidate will work primarily with hospice patients and their families, addressing a wide array of mental health conditions such as depression, anxiety disorders, and trauma-related issues. You will help clients process complex emotions surrounding end-of-life situations, facilitating conversations and interventions that promote emotional resilience and support.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of clinical experience in mental health, preferably within an outpatient or hospice setting.

• Proficiency in utilizing evidence-based therapeutic approaches, including but not limited to cognitive-behavioral therapy and solution-focused therapy.

• Strong interpersonal skills with the ability to communicate effectively with clients and collaborate seamlessly with a diverse treatment team.

Preferred Experience\n

• Experience working with hospice patients is highly desirable, allowing for sensitive and empathetic engagement with clients facing terminal illnesses.

Compensation and Benefits\n
