Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments that evaluate mental health, substance abuse concerns, and medical issues, ensuring they are sensitive to the cultural and age-related aspects of our diverse patient population.

• Provide individual, couples, and family counseling sessions aimed at improving emotional health, developing treatment plans, and facilitating crisis intervention as needed.

• Create and maintain detailed electronic documentation, including progress notes and evaluations, using our integrated digital health records system to ensure continuity of care and effective communication.

• Coordinate with community resources to offer patients and families access to essential services, such as medication assistance programs, social work support, and additional therapeutic services.

• Engage in collaborative efforts with Quality Improvement teams to optimize data collection practices, analyze service delivery, and enhance patient care outcomes.

You will serve a diverse patient population, primarily focusing on individuals and families dealing with end-of-life issues and hospice care. Qualifications\n

• Must hold a current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

The compensation for this position ranges from USD 115,000 to 130,000 yearly, reflecting your experience and expertise in the field.
