Key Responsibilities\n

• Act as a liaison between clients, their families, and outpatient providers, ensuring comprehensive care coordination.

• Engage with insurance companies to facilitate pre-certification and utilization management processes, enabling timely access to needed services.

• Participate actively in discharge and aftercare planning, providing transitional support that emphasizes continued client care.

• Deliver therapeutic meal support, modeling healthy relationships with food, and assisting clients in developing positive eating habits.

Compensation and Benefits\n

The annual salary for this position ranges from USD 90,000 to 105,000, based on qualifications and experience.
