Key Responsibilities\n

• Provide direct clinical social work services, including individual and family therapy, to hospice clients facing mental health challenges related to serious illness.

• Conduct thorough assessments and develop tailored treatment plans in collaboration with the patient’s care team to address the psychological, social, and emotional needs of clients.

• Actively participate in interdisciplinary team meetings to discuss client care, share insights, and coordinate effective treatment strategies.

• Offer crisis intervention and risk assessment for clients experiencing emotional distress, ensuring emotional support is prioritized.

Patient Population / Client Focus\n

Qualifications\n

Compensation and Benefits\n
