Work Environment

This role is set in an onsite home care facility, meaning you will be working directly within patients' homes. Key Responsibilities

As a Physical Therapist at Diamond Hearts Clinic, your core responsibilities will include:

• Conducting thorough assessments by consulting with patients about their physical conditions, medical histories, and specific symptoms.
• Diagnosing movement dysfunction to develop comprehensive, individualized treatment plans tailored to patient needs.
• Implementing therapeutic exercise techniques and instructing patients on proper usage to enhance recovery and improve mobility.
• Applying modalities such as manual therapy, stimulation, or massage to promote healing and alleviate pain.
• Training and assisting patients in the efficient use of assistive devices including wheelchairs, walkers, and other mobility aids.
• Maintaining detailed patient records to track progress towards goals and inform adjustments in treatment plans.
• Educating patients and their families about in-home treatment options, exercises, and lifestyle modifications to reinforce progress and prevent recurrence.

Patient Population / Client Focus

Our patients include children and adults from various demographics, each facing unique physical challenges. You will engage with clients who present with musculoskeletal disorders, post-operative recovery needs, neurological conditions, and other impairments. The emphasis on home care allows you to develop therapeutic relationships in a familiar and supportive environment, helping to build trust and commitment to rehabilitation goals.

Qualifications

The successful candidate will possess:

• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• A valid and active physical therapy license in the state of Florida.
• Exceptional communication skills, both written and verbal, to effectively interact with patients, families, and other healthcare professionals.
• Proficient skills in word processing and software applications pertinent to documentation and patient care.
• Flexibility and adaptability to execute therapeutic responses tailored to individual patient needs.

Preferred Experience

Candidates with at least 2 years of clinical experience in physical therapy, preferably in home care settings, will find themselves well-prepared for success in this role. Compensation and Benefits
