Key Responsibilities

• Conduct comprehensive evaluations of patients, including a detailed review of their medical history, physical assessments, and the determination of tailored treatment recommendations.
• Communicate evaluation findings and the proposed treatment plan clearly to both patients and their families, actively involving them in establishing realistic rehabilitation goals.
• Administer evidence-based treatment protocols that are responsive to each patient's unique health condition and changing health status.
• Maintain communication with a multidisciplinary team of healthcare professionals, addressing any changes in a patient's condition and adapting care plans accordingly.
• Document all relevant patient care activities promptly and accurately, capturing the course of treatment, progress notes, and discharge plans that highlight each patient's journey.
• Effectively manage time and productivity to meet organizational expectations while maintaining an organized schedule and attendance at scheduled meetings.
• Participate in marketing efforts to promote hospital and community initiatives, thereby enhancing public awareness of the services we provide.
• Engage in professional development by pursuing ongoing education opportunities and mentoring students in clinical practice.
• Uphold a commitment to excellent customer service and patient access, contributing to the reduction of barriers in home healthcare.

Patient Population / Client Focus

Our Physical Therapists work with a diverse patient population, including individuals recovering from surgeries, those managing chronic conditions, or elderly clients seeking assistance with mobility and daily living activities. Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid state license as a Physical Therapist in Florida or the ability to obtain one.
• Current American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers.
• Strong interpersonal skills with a dedication to collaboration and teamwork in a healthcare setting.

Preferred Experience

• A minimum of 2 years of clinical experience, preferably in home care or outpatient settings.
• Demonstrated ability to assess and develop customized treatment plans effectively.
• Experience in handling a diverse range of patient demographics and conditions.

Compensation and Benefits
