Key Responsibilities\n

• Provide compassionate and patient-centered support to individuals and families in the hospice care system.

• Conduct thorough assessments of emotional and mental health needs, utilizing evidence-based practices to diagnose and create individualized treatment plans.

• Implement therapeutic interventions, monitoring patient progress and making necessary adjustments to treatment strategies to effectively address changing needs.

• Collaborate with an interdisciplinary team to ensure comprehensive care, including regular meetings to discuss patient outcomes and share best practices.

• Engage in ongoing professional development to remain current with advancements in mental health practices and maintain licensure.

• Document all sessions and treatment plans accurately in our electronic health records (EHR) system, ensuring compliance with regulatory standards.

• Participate in case consultations and administrative meetings, contributing insights and recommendations based on clinical expertise.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals and families confronting end-of-life issues, providing vital emotional support and integrating grief counseling into their care plans. Your work will focus not only on the patients but also on their families, ensuring that everyone involved receives the necessary care and guidance during stressful times.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
\n
• Current Licensed Clinical Social Worker (LCSW) license in the state of California.
\n
• A minimum of 2 years of relevant experience working as a Licensed Clinical Social Worker, with a focus on mental health.

• Proficiency in delivering telehealth services, including familiarity with telecommunication technology and EHR systems.

• Extensive knowledge of mental health disorders, therapeutic modalities, and effective treatment methodologies.

Preferred Experience\n

• Experience in hospice or palliative care settings is highly preferred, as well as working with terminally ill patients and their families.

Compensation and Benefits\n

Core Mental Health and Behavioral Solutions offers a competitive salary ranging from USD 120,000 to 130,000 yearly , commensurate with experience and qualifications. Additional benefits include:
