This position offers a remote work setting, allowing you to carry out your responsibilities from the comfort of your home while collaborating closely with an interdisciplinary team of healthcare professionals. Although primarily remote, you may occasionally interact with patients in-person at our hospice center as needed. Key Responsibilities\n

• Conduct thorough psychosocial assessments for patients entering hospice care to identify their emotional, social, and financial needs.

• Provide individual and group counseling sessions, offering therapeutic support on adjustment, grief, coping mechanisms, and end-of-life transitions.

• Develop and implement comprehensive discharge plans, ensuring patients and families are linked to appropriate community resources that facilitate continuity of care.

• Collaborate with healthcare providers, nursing staff, and other professionals to optimize patient outcomes and enhance the quality of care delivered.

• Participate actively in hospital committees, bringing your expertise in social work to inform policies and social care practices within the institution.

• Maintain accurate, timely, and confidential documentation in accordance with hospital policy and legal guidelines, ensuring compliance in all aspects of service delivery.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 70,000 to 100,000 annually, commensurate with experience and expertise.
