Key Responsibilities\n

• Serve as a primary liaison between clients, their families, and various outpatient providers, ensuring a coordinated approach to care.

• Engage with insurance companies to facilitate pre-certification processes and manage utilization effectively, advocating for clients’ needs throughout.

• Participate actively in discharge planning and aftercare arrangements, helping to set clients up for success post-treatment.

• Provide therapeutic meal support, assisting clients in building a healthy relationship with food through modeling and educational interventions.

• Ensure every client receives empathetic and affirming care, attuned to their unique circumstances and intersecting identities, fostering an inclusive environment.

Patient Population / Client Focus\n

The LCSW will work primarily with adults and older adults dealing with a range of mental health challenges including anxiety, depression, trauma, and adjustment issues. By understanding and respecting the diverse backgrounds and identities of our clients, the LCSW will be able to create personalized treatment plans that address specific needs and goals.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current and valid LCSW license in Oregon.

Compensation and Benefits\n
