Key Responsibilities\n

• Manage a diverse caseload of clients, fostering rapport and maintaining professional boundaries while articulating a patient-centered approach.

• Conduct regular client meetings to evaluate their progress, ensure stability, and schedule follow-up appointments, including appropriate discharge planning.

• Develop, implement, and monitor individualized treatment plans in collaboration with clients, utilizing evidence-based practices tailored to their needs.

• Maintain accurate and up-to-date documentation and progress notes for each client in accordance with best practices and regulatory guidelines.

• Perform comprehensive intake assessments to identify clients' needs and formulate actionable treatment strategies.

• Conduct individual therapy sessions weekly, adapting therapeutic approaches as necessary to align with client progress and feedback.

• Participate in individual supervision to enhance clinical skills and professional development.
\n
• Collaborate with other healthcare professionals to coordinate care, share insights, and ensure thorough support for each client.

Patient Population / Client Focus\n

Qualifications\n

• A Master's Degree in Mental Health or a related field is required.

• Current and valid licensure as a Licensed Professional Counselor (LPC) in the state of Connecticut.
