Key Responsibilities\n

• Schedule and conduct initial assessment sessions for clients, delving into their personal goals, preferences, and treatment options.

• Develop customized treatment plans that align with clients' educational backgrounds, skill sets, physical abilities, and professional aspirations, adjusting these plans as necessary during therapy sessions.

• Collaborate closely with a diverse team including psychologists, occupational therapists, and other healthcare specialists to design comprehensive rehabilitation programs tailored to the unique needs of each client.

• Maintain meticulous case documentation including client histories, treatment progress, eligibility details, and communication records to ensure clarity and continuity of care.

• Monitor and evaluate client progress regularly, employing evidence-based assessments to determine the effectiveness of therapeutic interventions.

• Provide ongoing support to clients engaged in job training programs and occupational placements, swiftly addressing any challenges that may arise.

• Engage with clients' families and social supports to ensure a holistic approach to counseling and to identify resources to facilitate client care.

• Ensure timely assessment of client needs and the documentation of relevant reports to promote clarity in treatment direction and progress.

• Offer referrals and resources as needed, ensuring clients have access to additional support services.

Qualifications\n
