Key Responsibilities\n

• Conduct comprehensive assessments and develop treatment plans tailored to the unique needs of patients and their families.

• Facilitate pre-crisis and crisis intervention strategies, including behavioral guidelines and risk assessments, to effectively manage patient challenges.

• Document clinical interactions and treatment updates meticulously, adhering to regulatory standards and timelines.

• Utilize evidence-based screening tools as recommended by the clinical team to ensure quality assessments and interventions.

• Collaborate with interdisciplinary teams to create and implement positive behavior support plans aimed at enhancing patient care.

• Act as a liaison between patients, family members, healthcare providers, state agencies, and community partners to connect individuals with necessary support services.

• Engage actively in the licensing and accreditation processes, contributing to our commitment to maintain high standards of service.

• Establish and foster relationships with local, state, and federal agencies, and seek out potential partnerships and funding opportunities to support patient care programs.

Patient Population / Client Focus\n

In this role, you will specifically serve patients within a hospice care setting, focusing on individuals facing terminal illnesses. Your work will not only involve addressing mental health challenges but also providing emotional support to families during a critical period. As a trusted advocate, you will guide families through navigating available resources while addressing issues such as grief and loss, mental health disorders, or substance use challenges.

Qualifications\n

• A Master's Degree in Social Work (MSW) from an accredited institution.

• Current state license as an LCSW in good standing with the appropriate licensing board.

• At least two years of supervised clinical experience in a healthcare or community setting.

Compensation and Benefits\n
