Key Responsibilities\n

• Conduct comprehensive diagnostic assessments to accurately identify client needs and formulate individualized treatment plans.

• Deliver therapeutic interventions, including psychotherapeutic and psychopharmacological support, while consistently monitoring and adjusting treatment strategies based on client progress.

• Implement crisis intervention strategies as required, which may encompass safety planning, relapse prevention, and personalized referrals to local community resources for additional support.

• Engage with a multi-disciplinary clinical team to discuss and review client cases, share insights, and recommend adjustments to treatment plans to enhance effectiveness.

• Ensure thorough documentation and maintenance of client records, verifying that all information is complete, current, and reflective of the client’s journey.

• Facilitate coordinated care among clients, families, and external organizations to optimize treatment outcomes.

Patient Population / Client Focus\n

As an LMFT at SageWise Wellness, you will work with a diverse client base experiencing a range of mental health issues, including anxiety, depression, relationship challenges, and trauma. Our teletherapy model allows you to reach clients from different demographics, providing vital support in a flexible manner that promotes their journey towards recovery.

Qualifications\n

• A Master’s Degree in Marriage and Family Therapy or a related field is required.

• Active licensure in the state of California as a Marriage and Family Therapist (LMFT), Professional Counselor (LPC), or Clinical Social Worker (LCSW) is essential.

• A minimum of three years of clinical experience in the mental health field.
\n
• Proficiency in the diagnosis and treatment of a variety of mental health disorders.

• Strong capability in evidence-based clinical practices, focusing on outcome-driven care.

Compensation and Benefits\n
